RESUMO

This review describes individual studies of removable dental prostheses, evaluated their performance and clinical applicability for the proposal to expand health insurance coverage of titanium and its alloys to removable dental prostheses. Titanium and its alloys have become clinically applicable as prosthetic materials by improving dental casting systems. They have high biosafety and good mechanical properties, are excellent substitutes for the silver-palladium-gold alloys for casting, and are highly useful for removable dental prostheses. Therefore, the introduction of health insurance coverage for removable dental prostheses made of titanium and its alloys is worthy of consideration.

The aim of this study was to investigate the effect of repeated casting and heat treatment on the corrosion resistance of a commercial Ag-Pd-Cu-Au alloy as evaluated by electrochemical techniques. After repeated casting, the fifth cast of the Ag-Pd-Cu-Au alloy exhibited dramatic degradation of properties, although upon heat treatment, this corrosion resistance did improve. Despite the improvement by heat treatment, after five castings, this alloy may not have satisfactory hardness for clinical use. These results of this study demonstrate that, up to the fourth cast and heat treatment, the Ag-Pd-Cu-Au alloy has acceptable corrosion resistance and hardness.

In the context of optimizing dental care for patients who are elderly, the purpose of this in vitro study was to evaluate the surface gloss (with a micro-area gloss meter) of, surface roughness (with a compact surface roughness measuring instrument) of, and color change (with a dental colorimeter) in two commercially available injectable resin-based composites (Estelite Universal Flow (EUF) and Beautifil Flow Plus F00 (BFP)) as well as two glass-ionomer cements (GC Fuji II LC CAPSULE (FLC) and GC Fuji IX GP EXTRA CAPSULE (FGP)), before and after dental prophylaxis. After 24 h, the surfaces of each specimen were polished at 2500 rpm with a prophy brush (Mersage Brush, Shofu) and one-step prophylaxis paste (Prophy Paste Pro, Directa): under 100 or 300 gf load, and for 10 or 30 s, 4× cycles of cleaning. After mechanical cleaning, conditions were found for a significant reduction in the gloss level (EUF, BFP, or FLC; p < 0.05) and a significant increase in surface roughness (BFP; 300 gf load, 10 s × four cycles of cleaning). Overall, the longer time or higher prophylaxis load tended to decrease the surface gloss. However, the observed change in surface roughness varied between the restorative materials. There was no color change post-prophylaxis.

The purpose of this study was to investigate which were the most important factors in determining the location of the main occluding area by comparing its site on the dental arch between patients treated with implant-supported prostheses and those with a shortened dental arch (SDA). Twenty-five patients with Eichner B1 occlusion were enrolled in the study. The molar region in each quadrant was always edentulous. Fifteen patients were treated with implant prostheses, while the remaining 10 patients had SDA. Each patient was instructed to clench a piece of temporary stopping in the occluding area that was preferably used during mastication. The main occluding area was determined by locating the tooth on which the temporary stopping rested during clenching. The main occluding area was located ipsilaterally to the edentulous side in the molar region more often in the implant patients than in the SDA patients. The results of the present study suggest that the most important factor in determining the main occluding area is the presence of bilateral rigid molar occlusal support.

STATEMENT OF PROBLEM: Dental casting accuracy is influenced by the setting expansion of investment materials. Although setting expansion can help compensate for casting shrinkage, it cannot be fully realized under a confined wax pattern. Exactly how soft a wax pattern should be to ensure optimum setting expansion has not been determined. PURPOSE: In this study, the relationship between wax characteristics and the casting accuracy of a three-quarter crown was investigated. MATERIAL AND METHODS: Four different wax materials were used: paraffin 135 with a softening temperature of 37.5 degrees C (P38), paraffin 1080 with a softening temperature of 63.5 degrees C, Shofu Red with a softening temperature of 41.5 degrees C, and Shofu Hard with a softening temperature of 51 degrees C. Two mixtures of phosphate-bonded investment were prepared: one with 100% special liquid and another with 75% special liquid plus 25% distilled water. For both, the liquid/powder ratio was 16:100. A type IV gold alloy was cast into a three-quarter crown mold. The discrepancy at 6 locations (1 lingual, 1 mesial, 1 distal, and 3 facial) was measured with a traveling microscope. Five readings were collected. Means and standard deviations were calculated for all data. A 2-way analysis of variance followed by the Student-Newman-Keuls test for multiple comparisons was used to identify significant differences between groups at the 95% confidence level. RESULTS: For the gingival measurement sites (lingual, mesial, and distal), there was no significant difference in cast adaptation when Shofu Hard and paraffin 1080 waxes were used. However, the results with these 2 waxes were different than with Shofu Red and P38. For the 3 facial measurement sites, significantly different measurements were found for each wax; P38 demonstrated the best results. Casting shrinkage was smaller with the use of 100% special liquid. CONCLUSION: Within the limitations of this study, casting shrinkage was affected by the type of wax used and was sensitive to the site at which dimensional measurements were performed. The higher the softening temperature, the larger the casting shrinkage.
